AHCDPlanning for medical care in advance can help provide peace of mind for you and your family should you ever become unable to make decisions for yourself. Talking with your family and health care providers can help you create a plan that works with your goals, values, and wishes. Having an advance directive can lessen the burden for your family during an emergency and ensure your wishes are respected.

The state of North Dakota has some helpful tools to make this process easier:

North Dakota Health Directive

It is also very helpful to appoint someone to be your medical power of attorney and to make sure you have a living will - both usually require assistance from a legal expert. Give a copy of your advance directive for health care to your health care providers, power of attorney, family, and/or caregivers. You can change your directive at any time and it is a good idea to review it often.
