Mid Dakota Clinic Participates in Comprehensive Primary Care Plus (CPC+)***

Our practice is participating in CPC+, the nation’s largest-ever program to improve primary care.

Giving Doctors Extra Support to Help you Receive Better Care
Through CPC+, our insurers will give our practice additional resources to help us better manage your care. We hope to provide you the highest quality patient-centered care.

Additional Information for Traditional Medicare (Part A and Part B) Beneficiaries
To help us provide you with the best care, Medicare will start sharing some of your personal health information with us. This will provide us with a more complete picture of your health and allow us to better coordinate your care.

If you want to stop Medicare from sharing this information, please contact 1.800.MEDICARE as soon as possible.

Your Medicare benefits aren’t changing. You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it’s always your choice which doctors or hospital(s) you visit.

Questions?
If you have questions or concerns, you can call Rebecca B. at 701.712.4010 or 800.472.2113 ext. 4010, or bring it up next time you’re in the office for an appointment. You can also call 1.800.MEDICARE.

***Mid Dakota Clinic's Participation in Comprehensive Primary Care Plus (CPC+) Is Ending
The Center for Medicare & Medicaid Innovation (CMMI) is concluding the Comprehensive Primary Care Plus (CPC+) program for all participating practices effective December 31, 2021. We do not anticipate any change in the quality of care you will continue to receive at our practice as a result of this change.

Please note, the conclusion of our participation in the CPC+ program will not result in a change in your Medicare benefits. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it’s always your choice which doctors you use or hospital you visit. You continue to have the right to use or visit any doctor or hospital that accepts Medicare, at any time.
