Online patient portal
Our online patient portal - - allows you to communicate with us easily and safely according to your schedule. Using your own secure password, you can log into the patient portal 24 hours a day, 7 days a week from the comfort and privacy of your home and office.

Scheduling an appointment
You can make an appointment by stopping in at any of our locations, or by calling 701.712.4501 or 855.530.6001. You can also request an appointment online through My MDC Chart/Patient Portal.

When scheduling an appointment, please provide us with the general reason for your medical visit and the following information:
  • Full name (married and maiden)
  • Phone number (home and work)
  • Date of birth
  • Mailing address
  • Billing party's name
This information will be requested whenever an appointment is scheduled to ensure speed and accuracy in identifying your medical record.

If your appointment involves special services such as an insurance physical, a Workforce Safety and Insurance (WSI) examination, Veteran's Administration examination, or services provided through Social Services or the Public Health Service, please inform the receptionist so that these services can be properly arranged.

If you have been referred to the clinic by another doctor, please let us know so that we may keep the referring doctor informed.

Medical record
Your medical record is valuable because it provides a continuous history of your medical care. Your doctor may request that you transfer your previous medical record to Mid Dakota Clinic. The information in your record is confidential and cannot be released without your written consent.

Treatment of a minor
A parent or legal guardian must give informed consent for Mid Dakota Clinic to examine and treat a minor. If you cannot attend the appointment, then: (a) you may appoint another responsible adult to bring the child to the appointment; or (b) if your child is 15 or older, your child may come to the appointment without you.  You must, however, complete and sign the Authorization to Treat a Minor Form and return it to the Clinic on or before the scheduled appointment. You must also be available by phone, in case the provider needs to contact you.

Arriving for an appointment (late policy)
Mid Dakota Clinic providers strive to see patients at their scheduled appointment times. To maximize the time spent with patients and to minimize patient waiting time, we kindly ask that you arrive 20 minutes early for your appointment to allow adequate time for the registration process. If you arrive 10 minutes or more late for your scheduled appointment time, you may be asked to reschedule. We are aware that unforeseen events sometimes cause patients to be late, and we appreciate your understanding and cooperation with our late policy.

For your convenience, we offer the TODAY Clinic with no appointment necessary.

Monday-Friday, 8 am - 8 pm
Saturday, Sunday & holidays, 9 am - 1 pm
Open holidays except Easter, Thanksgiving & Christmas Day

Gateway Mall Clinic
Monday-Friday, 8 am - 5 pm

Patients at the TODAY Clinic are typically seen on a first-come first-served basis and rotated among the doctors on duty. To reach any of the TODAY Clinics, call 701.712.4200 or 800.472.2113 ext. 4200.

Canceling an appointment
If you must cancel your appointment, please call us as soon as possible so that another patient may be served during your scheduled time. We strive to reserve adequate time for each patient to ensure quality medical care and minimal waiting time.

On your first visit to the clinic, you will be asked to provide new patient information. This information will be used for establishing your electronic medical record and assigning a patient identification number. After your initial record has been established, please notify the clinic personnel of any changes in name, address, insurance, or billing information. To expedite the check-in process, please bring your insurance card.

We understand that your time is valuable, and we do our best to stay on schedule. However, medical emergencies may arise which might delay your appointment. Should such an emergency occur, we will do our best to keep you informed about the projected length of the delay.

Audio/Video & Photographs
Recording or transmitting any portion of an office visit, including ultrasounds, is strictly prohibited. This includes all electronic devices and all forms of media.

Follow-up visits
If you need a follow-up evaluation or treatment, please stop at the reception desk on your way out to schedule your next appointment.

Calling your doctor
Our nurses and other staff who answer your phone calls are trained to ask and answer pertinent questions. If necessary, they can arrange for you to speak with your doctor or assist with making an appointment. When talking with our staff, please have paper and pen handy for instructions.

Sometimes, the doctor or nurse is not available to speak with you immediately. Please leave a message following the recorded instructions, and a healthcare professional will return your call as soon as possible.

For a life-threatening emergency, call 911 or go to the nearest emergency room.

Lost and found
Please report lost and found items to the main lobby reception desk. The clinic is not responsible for the loss of any personal items.

Ample and convenient parking is available at all Mid Dakota Clinic locations. Spaces for individuals with special access requirements are designated in each parking lot.

Prescription refills
Patients sometimes ask why the doctor may want to see them before authorizing a prescription refill. Some medications may have side effects that can affect your health. Your doctor may want to examine you or conduct tests to ensure that your medicine is appropriate for your continued use. Your doctor or nurse can give you more information on the procedures to follow for your refills.

Service animals
Mid Dakota Clinic allows service animals only on the premises. Title II and III of the Americans with Disabilities Act (ADA) defines a service animal as any dog that is individually trained to do work or perform tasks for the benefit of an individual with a disability, including a physical, sensory, psychiatric, intellectual, or other mental disability. The work or tasks performed must be directly related to the individual’s disability. Tasks performed can include, but are not limited to, pulling a wheelchair, retrieving dropped items, alerting a person to a sound, reminding a person to take medication, or pressing an elevator button. Emotional support animals, comfort animals, and therapy dogs are not considered service animals under Title II and III and will not be allowed on the premises. Service animals must be properly leashed, remain at the handler’s side, and may not bark/growl at other patients, visitors, or employees.

Smoking is prohibited in all areas of the clinic and on clinic grounds to promote a healthy environment for all.

All weapons are prohibited on the premises of Mid Dakota Clinic even with a valid permit. Anyone with a weapon will be asked to leave the premises, with the exception of law enforcement officers if on official clinic business.

Wheelchairs are available for your use in the main lobby of each clinic. If you use a wheelchair to transport a patient to the hospital or adjacent clinic, please return the chair to Mid Dakota Clinic or notify us of where you are leaving the chair by calling 701.712.4500 or 800.472.2113.

Mid Dakota Clinic doctors will work closely with the staff of your chosen hospital if you require hospitalization.
