Bariatric Program Celebrates Landmark Procedure

St. Alexius Medical Center and Mid Dakota Clinic are proud to announce the completion of the 1,000th surgical weight-loss procedure through the Primecare Surgical Weight Loss Program. The program is only six years old, and the procedures are currently being performed by just two surgeons who also maintain general surgery practices.

"This is a big deal for us! We've been so successful because our low complication rate is similar to national rates," says Brandon Helbling, MD with Mid Dakota Clinic. "I credit our success rate to the comprehensive weight loss services we offer our patients. It's not just about having surgery. It's about knowing how to keep the weight off by making healthy food choices, living an active lifestyle and knowing where to turn for support."

Dr. Helbling joined the program nearly two years ago and is now the Medical Director. Dr. Gaylord Kavlie began the program in February of 2004 offering laparoscopic gastric bypass surgery. Today, the two Mid Dakota surgeons perform three different laparoscopic weight-loss surgeries: Roux-en-Y Gastric Bypass, Sleeve Gastrectomy, and Adjustable Gastric Banding. The 1,000th surgery was a gastric bypass performed by Dr. Helbling.

Hope White Bear was surprised to learn that she was the 1,000th patient to undergo weight-loss surgery through the Primecare Surgical Weight Loss Program. "It's the best decision I've ever made with my health. It's been so good for my whole family. I have no regrets, " says White Bear.

"There's nothing better than when a patient begins to see results. They're enjoying life again and doing things they couldn't do before. That's because of the emotional, physical and medical improvements we've been able to help them make," says Dr. Helbling.

"I'm still in shock about it. The entire staff was able to answer all my questions and made me feel very comfortable both before and after the procedure," says White Bear.

St. Alexius and Mid Dakota Clinic have been designated as Blue Distinction Centers for weight loss surgery, offering a comprehensive program which includes medical, surgical, nutritional, physical and emotional support for optimum outcomes.

See the related KFYR-TV News Story.

Surgicenter Receives AAAHC Accreditation for 3 Years

The Mid Dakota Clinic Surgicenter has achieved accreditation by the Accreditation Association for Ambulatory Health Care (AAAHC/Accreditation Association). Accreditation distinguishes this surgical center from many other outpatient surgical facilities by providing a safe work environment and the highest quality of care to its patients. Status as an accredited organization means that the Surgicenter has met nationally recognized standards for the provision of quality health care, set by the Accreditation Association.

More than 4,000 ambulatory health care organizations across the United States are accredited by AAAHC. Not all ambulatory health care organizations seek accreditation; not all undergoing the on-site survey are granted accreditation. "We believe our patients deserve the best," stated Marlene Sapa, Surgicenter Manager. "When you see our certificate of accreditation, you will know that the AAAHC, an independent, not-for-profit organization, has closely examined our facility and procedures. It means that we as an organization care enough about our patients to strive for the highest care possible."

Ambulatory health care organizations seeking accreditation by the AAAHC undergo an extensive self-assessment and on-site survey by the Accreditation Association's expert surveyors - volunteer physicians, nurses, and administrators who are actively involved in ambulatory health care. The survey is consultative and educational, presenting best practices to help an organization improve its care and services. "Going through the process challenged us to find better ways to serve our patients, and it is a constant reminder that our responsibility is to our patients and the quality of care we provide," said Sapa.

AAAHC accreditation is specifically for organizations that provide diagnostic or medical care on an outpatient basis where an overnight stay would not be required.
Mid Dakota Clinic First to Offer Digital Mammography in the Region

Women who undergo routine mammograms at Mid Dakota Clinic now have the latest diagnostic technology available to them, digital mammography. While the digital mammogram will feel identical to conventional screening, the filmless process expedites exam time and provides a more detailed image.

Mid Dakota Clinic is the first healthcare provider in the region to feature this state-of-the-art system, Selenia™ digital mammography from Hologicฎ.

Mid Dakota Clinic is very excited to be the leader in providing the newest technology for breast cancer detection, according to obstetrician and gynecologist, Jerry Obritsch, MD, FACOG.

With digital mammography, the radiologist can magnify the images, increase or decrease the contrast and invert the black and white values while reading the images. These features allow the radiologist to evaluate microcalcifications and zero in on suspicious or concerning areas. This allows more opportunities for the radiologist to make immediate decisions about the necessity for additional images rather than asking the woman to return to the clinic later.

"Demand for digital mammography is high, and women are excited and pleased that Mid Dakota Clinic now offers this state-of-the-art technology not only because it results in superior quality, but also because it usually requires less additional imaging time," says Dr. Obritsch.

Mid Dakota Clinic is committed to the fight against breast cancer. Patients who have questions about this important breast health procedure may call 530-6014 or 1-800-472-2113 ext 6014 for more information. Appointments may be scheduled at 530-5550, 1-877-530-5550 or online.

Breast Cancer Statistics
Breast cancer is the second leading cause of cancer death among women, exceeded only by lung cancer. Statistics demonstrate that one in eight women will develop breast cancer sometime in her life. The stage at which breast cancer is detected influences a woman's survival. If detected early, the five-year survival rate is 97%.

Dr. Bradley Now Offering Laparoscopic Hysterectomy

Dr. Shannon Bradley is now offering women a surgical alternative to abdominal hysterectomy. Laparoscopic Supracervical Hysterectomy (LSH) and Total Laparoscopic Hysterectomy (TLH) procedures are referred to as MIP's or "minimally invasive procedures", which require a shorter hospital stay with a faster and less painful recovery as compared to abdominal hysterectomy.

Hysterectomy is one of the most commonly performed procedures in the United States. Each year, hundreds of thousands of women nationwide undergo abdominal hysterectomy for abnormal uterine bleeding, fibroids (benign uterine tumors), chronic pelvic inflammatory disease, endometriosis and uterine or ovarian cancer. Total abdominal hysterectomy requires a large incision or "bikini cut" with the tissues of the abdominal wall being stretched to remove the uterus. Women typically spend three days in the hospital and require a six week recovery period. Supracervical and total laparoscopic hysterectomy by contrast, use small incisions where tissue is removed via laparoscopic tools. One to two day hospital stays with a less than two to four week recovery period is typical and make these procedures much more desirable for some women.

While there are still advantages to conventional forms of hysterectomy, laparoscopic hysterectomy offers an exciting alternative for some women. To make an appointment with Dr. Bradley, call 530-5550, 1-877-530-5550 or schedule online.

Dr. Jerry Obritsch Receives ACOG Community Service Award

Jerry M. Obritsch, MD, FACOG of the Mid Dakota Clinic Center for Women was recently awarded the 2008-2009 ACOG Community Service Award for District VI. The award was presented at the Annual Clinical Meeting of the American College of Obstetricians and Gynecologists in New Orleans, LA May 3-7. The American College of Obstetricians and Gynecologists (ACOG) established the Community Service Award to recognize a Fellow or Fellows who are currently or recently actively involved in a community program which is designed to help underserved or poorly served patient populations within the geographical confines of the District. Each District establishes its own selection process and submits their nominee for consideration to the Committee on Honors and Recognitions. Following committee review, the nominations are recommended to the Executive Board.

Dr. Obritsch started traveling to Standing Rock Indian Reservation in September 1991, after completing his residency in June 1991, at the University of Missouri - Columbia. Providing obstetrical and gynecological care to women in Standing Rock, Dr. Obritsch works closely with Indian Health Service Providers, resulting in high quality medical care. Standing Rock Indian Reservation is composed of 2.8 million acres and 8 districts in North and South Dakota, and is home to the Lakota and Dakota Sioux nation. Dr. Obritsch travels to Ft. Yates, ND or McLaughlin, SD on a weekly basis, providing prenatal care and ambulatory gynecological services. Many patients have difficulty traveling to prenatal visits because of weather or transportation issues.

In addition, Dr. Obritsch travels approximately 100 miles away from Bismarck to deliver approximately 120 patients per year in Standing Rock Indian Reservation. This distance creates logistical issues affecting a healthy delivery system. Through Dr. Obritsch, perinatal morbidity and mortality has been significantly reduced. He is now starting to deliver patients that he himself delivered. Dr. John Witt, Dr. Obritsch's partner stated, "Dr. Obritsch has single handedly reduced the perinatal morbidity and mortality at Standing Rock over the last 15 years."

Mid Dakota's Dermatologic, Cosmetic and Laser Center Offers Skin Cancer Treatment

Dr. David Appert, board-certified dermatologist and dermatologic surgeon, specializes in skin cancer treatment using Mohs surgery. Mohs surgery is a meticulous procedure that relies on the accuracy of the microscope, instead of the human eye, to help ensure that all cancer cells are removed when a skin cancer is treated.

Dr. Frederic E. Mohs pioneered the surgery at the University of Wisconsin in the 1930's. Mohs surgery requires the specialized skill and training of a Mohs dermatologic surgeon who acts as a surgeon, pathologist and reconstructive surgeon.

Initially, Dr. Appert functions as a surgeon to remove the visible tumor and a small margin of healthy tissue. Then he works as a pathologist when he meticulously "maps" the removed tissue and divides it into smaller sections to be examined under the microscope for remaining cancer cells. With the Mohs procedure, Dr. Appert can examine 100% of the tissue that surrounds the tumor. This results in a skin cancer cure rate of 97-99%. Other pathology techniques typically examine only a small portion of the tissue margin. Finally, he functions as a reconstructive surgeon in repairing the wound left by the surgery. Dr. Appert makes every effort to preserve cosmetic appearance and minimize scarring, and will help patients choose the reconstruction technique that is best for them.

Childhood Vaccination Changes 1/1/08

PROtect Kids ND
Childhood Vaccines Will be Paid for Differently as of Jan. 1, 2008

Starting January 1, 2008, children's vaccines will be paid for in a different way. In the past, the federal government provided vaccines at no charge for every child in the state. But starting January 1, because the number of recommended vaccinations has increased and the federal vaccine allotment has not, insurers will be billed for vaccines.

This means North Dakotans may begin to see - for the first time - deductibles, copayments and/or coinsurance for vaccinations on their explanation of benefit (EOB) reports from their insurers. Cost sharing amounts may vary depending on the insurance plan.

If your employer offers a flexible benefits plan, you may want to adjust your plan to include any out-of-pocket medical expenses that may not be covered by insurance. Check your benefit plan booklet for vaccination coverage as you prepare for enrollment in your 2008 flexible benefits.

Many health insurance plans cover immunizations; however, there are many variations from one health plan to another. Thus it is always advisable for members to contact their insurer to ask what is or is not covered in their health benefit plan.

If your insurance policy does not cover vaccines - or if your child is uninsured, Native American or eligible for Medicaid - please talk to your health-care provider. Your child may be eligible to receive free vaccines through the federal Vaccines For Children program.

Check your insurance plan's vaccination coverage to find out if your insurance company covers vaccines, check your benefit booklet or call your insurance company and ask the following questions:

Do I have insurance that covers immunizations?

Does my insurance cover these immunizations recommended by the Advisory Committee on Immunization Practices or the Centers for Disease Control and Prevention?

•	Diphtheria, tetanus and pertussis (DTaP or Tdap)
•	Haemophilus influenza type B (Hib)
•	Hepatitis B (Hep B)
•	Polio (IPV)
•	Rotavirus (RV or Rota)
•	Pneumococcal (PCV7)
•	Measles, mumps, and rubella (MMR)
•	Hepatitis A (Hep A)
•	Influenza (Flu)
•	Intranasal influenza vaccine (Flumist, LAIV or CAIV)
•	Meningococcal (MCV4)
•	Human papillomavirus vaccine (HPV)
•	Varicella (chickenpox)

Are there any coverage limitations or restrictions, such as age limitations on covered vaccines or restrictions on travel vaccinations? What are they? How do they apply?

Will I have to pay anything for immunizations? Copay or coinsurance? Deductibles?

Is there a maximum amount my insurance will cover each year for wellness services, preventive services or immunizations?

One of the most important things parents can do to protect their children is to make sure they are vaccinated against serious but preventable diseases. More information about immunizations is available on the North Dakota Department of Health web site at www.ndhealth.gov
